- This form is intended for residential Block Parties ONLY.
- For events which are open to the general public and/or will require blocking off streets in any of our commercial districts on Goodman, Galbraith or Hamilton, please contact the City Administrator's office.
- Applications MUST be submitted no less than two weeks (fourteen days) prior to the requested event date.
- Applicant must be 21 years of age, AND a resident of North College Hill.
- Fill out this application form completely. Incomplete applications will not be accepted.

Acknowledgements

City Resources*
Applicant acknowledges that depending on the nature of the event, additional fees may be incurred for the use of City resources &/or personnel, including but not limited to: Public Works equipment or personnel, event security detail, or standby Fire/EMS.

Noise Levels*
Applicant acknowledges that noise levels - including amplified music - must not disturb the peace of nearby residents and businesses. If City officials receive complaints about the noise, the Applicant will be warned. Failure to reduce noise levels after a warning may result in immediate termination of the activity.

Terms & Conditions
1. Any road block or other road obstruction shall be of a temporary nature and of such size and weight as to allow its immediate removal on demand of any public safety personnel or other public official having jurisdiction.
2. Any road block shall be placed and so obviously marked as to give adequate visible notice of its placement to any approaching user of the roadway.
3. All road blocks or other road obstructions must be removed from the roadway immediately upon the conclusion of the designated party.
4. Restriction of vehicular travel on the roadway shall occur only at the location described herein and between those times designated within the permit. If a road block or other road obstruction eliminates the only vehicular access for residents of the street, access for ingress and egress shall be permitted to the property.
5. A minimum of two (2) barricades will be dropped off the day before the Block Party to the applicant’s address. Barricades will be dropped off on Friday for parties scheduled for Saturday or Sunday. Barricades will be picked up at the applicant’s address on the first business day following the party. If additional barricades are needed, please indicate the number requested here: ______.
6. There will be no fee for this permit. Any damage done to City property, including but not limited to barricades, cones, or other public property shall be the responsibility of the applicant.
7. Applicant is also responsible for ensuring the right-of-way area is cleaned. Anti-littering laws shall be observed.
8. Applicant shall be responsible to contact an adult member of each affected household and receive consent to host a Block Party on the date and time as indicated on application from at least 80% of residents who live in the block or street affected. Applicant shall inform residents that the street will be blocked off.
Agreement*
I (we) have read and understand the terms and conditions of the Block Party Application and agree to abide by said terms and conditions. I (we) understand that the failure of any person participating in this designated activity to abide by the terms and conditions or conduct themselves in accordance to North College Hill Ordinances or Ohio Law shall result in the revocation of this permit. Applicant further agrees to hold harmless the City of North College Hill its employees, officials, and assigns from any claim for damage which arise for any reason of the issuance of this permit.
